I am an RN, with a Master's degree in nursing and a certification in hospice and palliative nursing. The past 25+ years of my nursing career have been with patients & families that are facing end of life. I have worn many different hats in my nursing career, from acute care hospitals to home health and hospice.
My career in hospice started as an RN Case Manager, then progressed to Supervisor, Manger, and Director of Patient Care Services.
I have a passion for End of Life Care. At this point in my career I believe I can be of better service to patients and families by working independently with those approaching end of life. I have worked for several hospice organizations, of which all have been extraordinary experiences. My goal is to share the knowledge and experience I have gained over the years to support you and your loved ones walk through this often challenging time together. I am very open to coordinating with your current MD or Hospice Team. However, being on hospice is not a requirement.
My Goal is that you spend this final chapter to the best of your ability, surrounded and comforted by those you choose. It is important to me that your voice be heard and your wishes be upheld. Equally important is that the stigma and fear surrounding this final chapter be diminished for you and your loved ones. My promise is to always to be honest with you. I'm sure we will cry and laugh together as we journey down this road
I will be your advocate.
My approach is centered around helping my patients/families identify what is most important to them during this stage in their life and to know what their options are so they can make informed decisions. I am committed to providing a safe and non-judgmental space for my patients to explore their inner world and work towards their desired outcomes.
I can assist with Advanced Care Planning/Directives, discuss why a POLST is important (Physician's Order For Life Sustaining Treatment) discuss cremation, different types of burial services, in home funeral services, memorial services, and A Celebration of Life Gathering.
Discuss your disease diagnosis and prognosis with you. Attend MD visits with you if you wish. Hold family meetings. Explain what to expect as your disease progresses. Create a legacy project about your life. Discuss anything related to death and dying. And as time draws near I will create a loving, peaceful, environment for you when you are ready to leave this world. I can recommend bereavement of other services for your loved ones if asked.
